About people dying in the stairwell area. I think I understand what their problem was. When you get to the end there (the spot with the leaky pipe), you have to "turn around" to trigger the "cinematic overture". Otherwise you just get beaten repeatedly until you're dead. Most likely many gamers didn't know what to do at that point. Anything before that should have been no problem and I'd be quite surprised if it were.
